Why the tape measure around your waist is a better predictor of cardiovascular disease and visceral fat than BMI or total scale weight.
The Superior Health Metric
While athletes obsess over body fat percentage to track physical appearance, the medical community relies on the Waist-to-Height Ratio (WHtR) to assess metabolic risk.
The logic is simple: where you store fat is infinitely more important than how much fat you store.
Subcutaneous fat (fat under the skin, like on the thighs or arms) is relatively benign. Visceral fat (fat stored deep within the abdominal cavity, wrapping around the liver, pancreas, and intestines) is highly metabolically active. It releases inflammatory cytokines and is the primary driver of insulin resistance, type 2 diabetes, and cardiovascular disease.
The Formula
WHtR = Waist Circumference / Height
(Ensure both measurements use the same unit, e.g., centimeters).
How to Measure Accurately
- Stand up straight and exhale normally. Do not suck your stomach in.
- Locate the midpoint between the bottom of your lowest rib and the top of your iliac crest (hip bone). This is typically right at, or just above, the umbilicus (belly button).
- Wrap a flexible tape measure around this circumference. Ensure the tape is perfectly horizontal to the floor.
- Record the measurement.
The "Keep Your Waist to Less Than Half Your Height" Rule
Extensive epidemiological studies (including data spanning 300,000+ adults across multiple ethnicities) have concluded a universal boundary for metabolic health: your waist circumference should be less than half your height.
This equates to a WHtR of 0.50.

Why it Beats BMI
A heavily muscled powerlifter might be 1.75m tall and weigh 100kg. Their BMI is 32.6, classifying them as Class I Obese.
However, if they have a waist circumference of 82cm, their WHtR is 0.46 (Healthy). The WHtR successfully identifies that their excess weight is muscle, not dangerous visceral fat. Conversely, a "skinny-fat" individual with a normal BMI but a distended belly (high visceral fat) will be accurately flagged as High Risk by WHtR.
