Skinfold Calipers vs Ultrasound

The Evolution of Subcutaneous Measurement
For decades, skinfold calipers have been the gold standard for tracking subcutaneous fat (fat immediately under the skin) without clinical machinery.
However, calipers require a technician to physically pinch the skin away from the muscle. This introduces human error (inconsistent pinch depth) and can be physically uncomfortable for the subject.
Enter A-mode Ultrasound.
How Ultrasound Measures Fat
Consumer and semi-pro ultrasound devices (like the BodyMetrix) use high-frequency sound waves. The device is placed flush against the skin (using gel) and emits a pulse.
Different tissues have different acoustic impedances. Fat, muscle fascia, and bone all reflect sound waves differently. The device measures the time it takes for the echo to return, calculating the exact millimeter thickness of the fat layer down to the muscle fascia.
The Advantages of Ultrasound
- No Pinching Required: Ideal for obese individuals where a skinfold pinch is impossible, or for highly muscular individuals where the skin is tight and difficult to detach from the fascia.
- Measures Intramuscular Fat: High-end ultrasounds can detect fat marbled within the muscle tissue, a metric usually reserved for DEXA scans.
- No Tissue Compression: Calipers physically compress the fat layer (which varies based on hydration). Ultrasound measures uncompressed tissue.
The Drawbacks of Ultrasound
- Cost: A good Harpenden caliper costs £200. A clinical-grade ultrasound unit costs £1,500+.
- Operator Error (Angle): The probe must be held perfectly perpendicular to the tissue. If it is angled even slightly, the sound wave travels a diagonal path, artificially increasing the recorded fat thickness.
- Algorithm Dependency: Just like calipers, the raw millimeter data must be fed into an algorithm (like Jackson-Pollock) to estimate total body fat. The ultrasound only measures thickness at specific sites; it still has to guess the rest of the body.
Which Should You Use?
For the vast majority of athletes and coaches, high-quality calipers remain the superior choice due to cost and accessibility. The variance in caliper tracking (if done by a consistent technician) is perfectly adequate for tracking week-to-week fat loss trends.
Ultrasound is a fantastic clinical tool, but unnecessary for personal tracking.
